I’m using a scroll button mouse with my Mac (not a 2-button Windows mouse). Neither clicking the mouse or the scroll button, nor doing this in combination with the command, alt or shift keys, allows me to “paint” an outline. What am I doing wrong (besides, probably, not using a 2-button mouse)?

I doubt you are doing anything wrong, but I don’t exactly know what kind of mouse you are describing. Could you possibly link me to a picture of it? If I can find the mouse I can try to reproduce the issue and resolve it.
